Our practice now offers Telemedicine (please check with your insurance for benefits and eligibility). The following are links on how to access and connect. 

1. Please bring at your visit your current insurance card, photo ID and new patient packet filled out and signed.  

2. Medical Records:  Bring your up-to-date medical history, prescription list. If you don't have your medical records on hand you can sign a Request for Medical Records form in our office to request your medical records from your previous physician. Please have available the physician's name, phone number, fax number and ID number (if needed) to promptly request your records.

1. Parents or Legal Guardian please bring your photo ID, patient's current insurance card and photo ID, and new patient packet filled out and signed.

2. Bring immunization record at every visit.

3. Parents or Legal Guardian must accompany minor patient's during office visit.

1. Please update your insurance information, phone, address, billing information, pharmacy, etc. before your appointment or when you arrive.
